Like your Indy's I have W5's with the offset rockers and unless you are very careful and pay really close attention the pushrods will slip out of the cup easily when putting it together and when setting the lash.
It's possible that with just the right harmonics in the valvetrain the pushrod could flex just enough to cause it to slip out.
