Probably happened with a pumpkin swap. They fell out and didn't get noticed. You can see what happened next. Ouch! Are you running Original adjustable bearings or Greens?
My thought too. Post a picture of your axle bearings. If they are the OEM style taper bearings, then you will need to replace those thrust buttons. If they are the Green style or sealed bearing style then you do not need them.
