I had this on my car maybe 200 miles. Top pic is the upper spacer and other is the heatshield.

Did you notice the lower picture where the inside of the shield is falling apart?
 
You should contact Quick Fuel. Not only will they make it right with you, but you may alert them to a defect that may not be limited to your part.
 
I bought one at Carlisle. I couldn't get the thing to idle clean. New gaskets on both sides. Back to a single gasket and AOK.
 
AFB/ factory cast manifold

For me, never on a mopar and not anyone I know.
Seen issues on chevies,gm,ford though.
I never really use the iron manifolds, except once, no issue.
Air cleaner type, gasket thickness, and fan/shroud style can be key to issues like that. Heat has to go somewhere and you definitely do not want to trap it under the hood. Less clutter the better, like **** all over the fenders, big shrouds etc.

I havent seen it all, I'm just lucky I'm sure.
 
I have one on my Duster. Mine came from cool carb technologies, i believe he was the original designer, patent one. My carb stays 30 degrees cooler than the intake, thats huge. Pretty simple really a composite fire retardant heat resistant material sandwiched between two plates - Home
 
My carb was cooler with it, but it wasn’t so heat resistant in my case. Deteriorating and falling in my engine.
