From reading around it sounds like this factory hole is used and modified slightly to become the new hanger mount? Does that sound right?

That's what I see too. A collar is welded in the hole for strength and to prevent the rail from collapsing. Look through the photos. mopar rear spring relocation kit - Google Search:
