Don't have the info in front of me but this is the kit sold by crackedback on here. looks factory, no heatsinks simple relay system with proper connectors and wiring, when aimed properly, rivals present day vehicles.
The heat sinks are part of the bulb assembly if you get LED. I got a DIY kit from slantsixdan and used GE Nighthawks recommended by both him and crackedback. Very happy with the results
