By the way you can install these pretty much with homemade tools. You need some oil, and a good quality bolt to crimp the device, and a "through hole" with a handle for a bushing. That needs to be hardened, you can even use an oversize nut and hold it with a wrench. So the order would be bolt....some hard washers, your nut, more washers, and the insert. Holt the nut, twist the bolt.

I ordered a bag of 25 through Fastenal, I will pick them up on Tuesday, and yes I plan to do the bolt-nut-washer trick to pull them into place
 
Not that I know of. So far as I know it originated up here. The owner is aging and not very active at all anymore. I'm 71, part time, I just build electronics and electrical components. You can find some videos of some of our stuff
