Manifold to downpipe bolts

How do ya'll keep the bolts in place when connecting exhaust pipes? They are a real pita to do by youself. Thought about tacking bolts to manifold?
 
They had keepers to keep the bolt heads from turning originally.

Dodge dealers still sell a version of them, part number below.

I used an rubber "O" ring /washer it crushes / burns off // handy when the bolt is upside down !!!!
 
I like to use somewhat undersize bolts and lowest (IE grade 2) grade bolts I can get, so I can twist them off if I need to next time they come apart. This of course does not help holding them in place, but I've always managed to "rassle" them in there
 
The keepers I mentioned above are steel.

They were intact on my (then) 49 year old Valiant when I had to drop the Y-pipe to install a new oil pan gasket.

They held when I removed the nuts from below with my 1/2" drive impact.

So how 'kinda permanent' are you after? ;-)

I weld a tab on either the nut or bolt head.
 
This is what I did on my e body exhaust manifold,added some flat stock to the exhaust head pipe bolts as an anti-rotation device so you don't have to fight putting a wrench on the bolt head, just use a socket and long extension from under the car, works out great.

I used a stud on the drivers side the rear bolt and nut it on the pipe flange. Then just need a nut to complete fastening it from the top. There's not a lot of room to get the nut in there if using a bolt from the top. I'm using 340 manifolds and tti 21/2 pipe, which makes it even tighter. To be truthful I hate working on my Duster compared to working on my Hemi Cuda.
