Current setup has a small PCV on the valve cover passenger side rear. Front passenger side has a plug grommet. Driver side rear has a grommet with push in filter and front is the oil filler cap.

The ME Wagner is larger inside diameter than the current grommet PCV location. I tried a 1.25" wide outside diameter grommet but it doesn't cover the taps. I need a wider grommet. Mancini has a "wider" grommet. Is that what you guys are using?

I know exactly what you need for a Wagner PCV to MP cast cover and am pretty sure you won't find the correct aftermarket grommet at most stores.

Order a Ford Performance Parts Valve Cover Grommet Set M-6892-F from Summit or similar. Fit and works perfectly.

I see what the prior owner did. He found a wide enough grommet that mostly fit and he used black sealant to hold it in. Instead of using the 2nd oil filler cap. Most likely to have a shorter vacuum hose to run at the rear of the throttle body. Are you guys running two oil filler caps?

My suggestion worked with BB cast covers that come with a breather/filler hole already knocked out. The opposite end is not (just a blank) like you see in these pics. I've got the pcv in the notched breather hole on right side, and a twist-on filler/breather cap on the opposite cover.
