Maintenance Card ?

Old Great ones please educate me !!
This was in a plastic sleeve under hood on fender well , I have to assume its something modern ?
Know way that plastic sleeve survived since 1966 , my new purchase had it and basically curious so what info on plate pertains to , thanks Chris
