I dont know what you are missing but I have this to offer. On a tapered fitting, never back the nut off to align the hole for the cotter pin. Always go tighter. I cant say I have ever used a torque wrench on castellated nuts. Make sure the mating surfaces are dry and clean. The taper is what holds it together.
