It's based on the fact that private insurance can be more efficient than government run Medicare. On the same amount of money, they will give you what they promise (usually) and money back (sometimes).
Look up Medicare advantage programs. My Mom has one, and is pretty happy with it. But be sure to shop around and get as much information from as many sources as you can.
