I changed a light bulb above the toilet yesterday, and I THOUGHT that I had sufficiently tightened the three set screws that hold the heavy glass fixture on. While my wife was at the lavatory, the fixture fell, crashing on the commode seat and sending glass all over the place. Just moments before, my wife had been squatting on that seat. It might have killed her, falling from a 9 ft. ceiling. I've done some bone-headed things before, but this one could have been really serious.
