I don't have a part number or manufacturer, but I recall it being ACC carpet. It should fit your '69 Dart, the rule of thumb for the Dart is '67 - 76.
Correct, column shift with a bench seat. If you have a console, just cut the carpet where you need for your shifter hole and console brackets, and it'll fit.
