I had this happen to me once just when putting the pistons in. The ring compressor wasn't low enough on the piston and the bottom oil ring popped out and got stuck between the piston and cylinder wall. Folded the ring over pretty good and made it hard to turn the assembly over, but it never ran that way. Caught it early enough to pull it back out and swap it with a fresh one.
