If it is flat you can get it made at a local glass shop. You can't the the factory blue/green AC tinted color but for clear take examples of the old and they will cut and have it tempered.

The slants my machine shop buddy built for me where similar. He bought 3 different gear/chain sets and they were all off by a half tooth. We degreed the cam and it was dead on so he marked the gears to be aligned where it was. Yes it was a 1/2 a tooth off from the (aftermarket) factory...

Yes, need to see the hinges. If they are the '63 to '65 style the lower hinge steel is HARDENED, HARDENED and more HARDENED. I personally would send it to Leo if I do another one. You need matching hardened bits to drill it out. Painful without the right tools.
