It is not a "relocation of the wrist pin". It is a change in the distance from the wrist pin centerline to the crown of the piston that changed. Also known as 'compression distance'.
Whats the milage on this truck? Did it have a "K n N" type air filter on it? The 5.9 and the 6.7 head and or block frequent my shop for some form of failure every week.
No. that 'visible horse shoe' area of the the cylinder head is just a 'void'. nothing goes there except lost carb e-clips, small nuts, bolts and washers. It's just a 'heat separator' from the rest of the head.

Lets have a look at the engine side of the flywheel and the front side of the harmonic balancer, what I see is a "cast" crankshaft, other things have been modified maybe they got the balance addressed too.
Do you have any measuring tools? Otherwise, from just the the pictures, there's an off-set built in, so that rules out an LS or 4.6/ 5.4. So more info possible?
The scruffy intermediate shaft is common for an original shaft. The noise? Maybe it's not the engine, maybe it's the transmission and just transferring the noise up.
