70 Dart Decode for Restore

I've found the broadcast sheet in the back seat a couple days back. I can't for the life of me get this thing decoded. I've got a few codes figured out but would love to get extra help.... can someone please decode the broadcast sheet for me. HUGE THANKS IN ADVANCE.
