Does this MP DP 4150 flange intake actually exist?

Looks like the same casting as Action-Plus 8007 with the Mopar branding. Was this the original issue P4876335 before the (part number change) bastrardization transformation P4876335AB that looks entirely different?

That manifold looks like the one in this Mopar Performance catalogue - it is the 4th one listed. It looks to be made by Weiand. I zoomed into the catalogue picture. Note number 8015 on the runner.

I have the same intake although mine says Weiand and not Mopar. It has the same 8015 on the runner. Bolt pattern is the W2/W5 pattern. Port size is the same as a standard LA head.

A lot of work to match to a W2 oval port but less work to match a W5.
