Many countries do not have the same style guardrails that we do here. Ours have the ends embedded in the ground so that an oncoming vehicle does not get impaled, like you see here.

RIP. Sadly and shamefully there is a guard rail system manufacturer here
in the PRK whose design causes these horrific results despite their
assurances the contrary. The Calibrokia Attorney General's office has been
going after them but it won't help those who've already been impaled.

I don't think there was time for an ouch. Since there appears to be blood on top of the guard rail just inside the car, I bet they felt no pain. At least I hope not.
