904 convertor identification

So, long story short I bought this convertor probably 10 years ago(used), shoved it on the shelf and never used it. Looks like CSSX5 but google turned up nothing for me.

Can anyone ID the numbers on it? Or point me in a direction to go hopefully. Thanks, Chris.
