Clutch Noise

Here's a short video of a noise when clutch pedal is coming back up. What is it..?
Not in this vid, but there was a constant noise when pedal was depressed that seemed to get louder the longer depressed...and same type noise once pedal was up but in gear and moving...like clutch was slipping..? I adjusted clutch rod and that seemed to help the noise when pedal was depressed, but "slipping" noise remained. Can you adjust a clutch slip by the rod..?

Sounds like the springs on the disc are hitting the flywheel bolts right before engaging. You may have the wrong flywheel bolts ,Heads to thick or the flywheel cut to thin.
