Cold starting.

Add a block heater. Their only about $25 at NAPA. In the cold months I plug it in about 4 hrs. (or the night before) I plan on going somewhere. In the summer months 2 hrs. is plenty of time to warm it up. I put mine in the drivers side front lower freeze plug (down and behind the PS pump) and ran the wiring under the battery tray out through the core support and tuck it in behind the bumper when not in use
 
Mine has a milled airhorn. It starts easy. couple pumps, fire, stay on high idle maybe 30 sec and then it will idle. That cam if I remember correctly is all kinds of nasty though. Might be causing some of the issue.
If you don't have a choke you shouldn't have a working fast idle cam. They both work together,
