Good morning friends and members fender tag help please

I have a friend here in town that has a 68 roadrunner, he is almost wheelchair bound needing work done now that he has insurance.
Well he asked me about this tag and I told him I could get it figured out what his car was equipped with.
Any help would be great, even just a site I can go to to figure it out for him, hope to get him happy.
