Guitar wiring diagram?

I dug out one of my old guitars that needs rewiring. It is a Cord with 2 single pickups, one double, 2 tone dials, 1 volume, and a 5 way switch. Anyone have a diagram for something like that? It has been atleats 8 or 9 years since I even looked at the back of this thing so I forgot everything. Good thing my Ibanez is ok, it just needs new strings.
