I have two questions that I am in hopes someone will answer.
1. Did the 1966 Formula S ever come with no stripe down the middle?
2. Did the Formula S ever come with these type of rims/caps?

Rallye rim came out in 1970, so no on that.

Not sure on the stripe, but i bet it could have been deleted from the package if car was special ordered.
 
There are no experts, there is way too much for all of us to learn yet. :)

The 1970 model year was the very first year that Rallye wheels were available.
 
My 65 Formula S didn't have the center strip. I believe it could be ordered either way. 65'

1... The Stripe was an Option... You could order the stripe as an option Even if you were buying a Slant 6 !! Like in 1965, it did "Not" come standard... even on the Formula "S" !!! In 65 the cost of the stripes (option) was $31.25 !!!! 66 was probably about the same price !!

2... Was already answered Correctly with a "No"....
