Upper hose has this ID which crosses to a belt: 6K21 EBP 80M-150. For the life of me, I can't find a replacement. Current lower is a flex hose (Gates 25478). I'd like to go to a molded hose if I can find the right one. I can't find this Rad (2949015) in the listings but believe it's for a 225. It's been modified to have the outlet on the drivers side for the 360 water pump. I may just have to find a similar belt and cut to length if I can't cross the upper 6K21 Hose.

OK, we are dealing with more superseded numbers which quite often don't show up in the parts catalogs.
In 1969 part number 2949075 is listed in the Chrysler parts catalog. It's a 22 inch radiator used with 170 or 225 engines.
2949075 supersedes to part number 2949014, while the number 2949015 is found stamped on the same radiator.
Not all but some radiator part numbers will be different from the stamped number by one digit.
As far as what hose to use, it maybe a trial and error fit.

I agree on the trial and error process....
I've got an email into Gates to try and identify the hose currently being used. Hopefully they can cross it. None of the local auto shops (nor I through google) have been able to cross the 6K21 EBP 80M-150. I'm sure I can find something close that I might need to "cut to fit".

Found this much older thread. Not sure if the member/guest is still active. Post shows the same radiator being used with a 340. Would love to hear from anyone that knows what upper hose he's using. Looking for a Fan Shroud?

I do not see any modification.

Slant radiator was set up for hose connections offset to the right.

You say the engine is a 360, with pump inlet on the right. So yes you can get away with using this radiator.
What you realy need is a parts store that has the hoses hanging.
When I got my car (67) it had a later square tank slant six radiator.
I have one note scribbled saying the connections were 1.5" diameter and the length needed is about 20"
Anotehr note that I later used upper hose 21202 with 1.75 to 2" cut off from each end. That's probably a Dayco number.
I can not guantee the later square tops put the connections in the exact same location as the bubble top radiators.
