The one with the Sure Grip instructions was a 1964 B-body Dodge 2 door Sedan (post). That year the various submodels were 330, 440, 500. Mine was a 440 which precedes the 440 Cu In engine and had to do with trim level not engine size.

OK, thanks, so I am still looking for confirmation on whether the decal was used in 1969.

I have these for the emblems on the quarter panel, off of my 69 Dart GT. Sorry the drivers side is hard to read.

At least on the 69 the holes are right above a straight body line for reference!
