This is a card for a rare 1969 Polara 500 Convertible with the HP440. This card does not have options on it, it is labeled Locator, not sure if there are different cards.
This card does not define the columns on the right but this card decodes as "LOCATOR".
This is the only 69 card I have ever seen, it belongs to a car I am decoding for someone.

A bit of new decoding information...

if there is a punch in the R X or 0 row and another punch below it that represents a Letter of the alphabet.


Example #1

R+4 =D
R+4=D
1=1

DD1 is my paint code (Lt. Blue Poly)


Example #2

X+7=P
4=4
0+7=X
P4X is my trim code (GT Trim package)

The way it works:
The R row and a number below it

R+1=A
R+2=B
R+3=C
R+4=D
R+5=E
R+6=F
R+7=G
R+8=H
R+9=I

The X row and the number below it

X+1=J
X+2=K
X+3=L
X+4=M
X+5=N
X+6=O
X+7=P
X+8=Q
X+9=R

The 0 row and the number below it

0+1= not used
0+2=S
0+3=T
0+4=U
0+5=V
0+6=W
0+7=X
0+8=Y
0+9=Z
