My book doesn't break down what all is included in the Formula S package but headrest(s) were an option in 67 for cars with bucket seats. Option code 531 for the drivers side and 532 for the pass side. That's right, you could get a Cuda with just one headrest on the side of your choosing or get a set. Cost was $20.95 each.
 
67 was the first year the a-body seat frames would accept head rests. Only convertibles got them though unless special ordered.
 
Whats correct for 67 is a plastic lock strip for the gasket that looks like chrome.
The reveal mouldings for 68 and later are stainless steel.
front windshield reveal mouldings can come from any A-body except Duster/Demon/Dart Sport.
