Does it need to be certified? Could you make your own? You could use the bell for a drill pattern and make the "blank" and I could trace you the outline of the starter area and first couple of holes for location on paper?
 
If you would be willing to.... just trace the outline of the plate and the two large holes, I could then make one. That would be awesome.
