My trunk extension is like swiss cheese in one area. Can I change that panel without disturbing the adjacent panels? Would I be better to also buy the lower quarter patch panel and cut that off to better access the trunk extension?

I would be surprised if the quarter skin is not rotting away from the inside if the trunk extension is gone. If your hands are small enough you may be able to feel between the panels or get an inspection mirror in there to see. But yes, not having the quarter skin in the way makes replacing the extension easier, but not necessary.

Most cases only the turned down portion of the trunk extension is rotted away. That portion can be replaced from underneath the car. Doesn't have to be factory stamped metal either. Most any sheet metal would work. It does enclose the truck.
It goes to the bottom of the quarter for 2 reasons,
So that panel doesn't flop in the breeze,
So they didn't have to weld it up high on the skin and hide those welds.
