Six or four special washers on each exhaust manifold?

The older small block exhaust manifolds use special washers that allow expansion under the bolt/nut head. I thought I had read in a factory service manual or elsewhere that the two center bolts did not use these special washers, only the two fasteners on either end. However, on Ebay there is a fastener kit that included a special washer for each and every fastener. What is the correct application and why? Thank you.

The two nuts did not use the washers I believe.

In the drawing it shows no washers under the center, but in the picture you can see that there is washers under the center bolts. Take your pick I guess
 
They're called conical washers. I ended up using them on all the studs except 1 because the stud was too short. Here's a thread I started asking basically the same question....
