    yeah, them washers are on backwards. Probably the reason for the failure. It's pretty obvious that in that orientation the washer is just going to spread the bushing as force is applied. I actually went out and looked at mine:)

    It doesn't take an alignment shop eval to see that. Those mounting hole are shot. The bolt that goes through has d flats for the eccentrics so they don't turn and once you loosen the bolt the eccentrics are supposed to move the upper control arm in or out against the flat that are flattened...

The hole for the eccentric bolt should only be the size of the bolt and the eccentric makes the changes needed by pushing on the tabs that were flattened.     I wouldn't think the norm is to grab it by the lower arms since they are a part of the front suspension. K-frame or bumper mounts would have been better. I would have the alignment checked just in case.
