Need help wiring a fan relay

I have a fan relay and plug,got no instructions.It,s a 5 wire plug and relay,need help wiring in to my 66 Dart.I also have an Audi rad with dual fans which have only 2 wires coming from each fan.The 5 wires on the fan relay plug are Red
White
Yellow
Black
Blue
I,m no wiring guru and don,t want to fry the relay,so any help would be appreaciated.Thanks

the guy that runs that site owns a local street rod shop.. he is awesome at wiring... look around the site he is constantly adding info..
