Bad noise when clutch is disengaged! Help

I would still guess it is the input shaft bearing on the transmission and possibly the input bearing retainer shaft that the throw out bearing rides on has excessive wear. Jacking it up may have changed the place the bearing is riding.
I ran in to issues that were similar with the old Chebby truck 4 speed trannys.

Agree with inspecting the bearing retainer.
It can be cracked and flex enough to cause alignment problems.
But, by looking at your pic, it looks as though the adjustment is pushing the TO bearing too far in and seems that the weights have a wear scratch on all of em. do they? if so then that`s part or maybe all of the problem.
