HR-135 Horn Relay

Does anyone know how to connect this? My horn relay went bad in my '66 Dart and the replacement connector terminals don't appear to be in the same location as the original. Not sure which wire to connect to which terminal...
The three terminals are labeled "1" "3" and "5."

1 goes to the + of the horn
3 is +12v
5 is the horn wire from the steering wheel.
When the relay coil is energized (5), the switch from 3 to 1 is closed, allowing +12v to go to the horn.

Thanks! Horn works now...in fact, too well. Won't stop honking. I guess I have to get back into the steering column and figure out what went wrong with the turn signal switch and Grant steering wheel installation
