Well for me it was a muscle in my back sorta between my shoulder blade and my spine. It would cause me to have pain right out through my front right where your heart is so I would think I was having the big one! I was to my doctor many times told him I thought I was having a heart attack the pain was so bad .He sent me for 3 ekgs all said my heart was normal I finally figured out it was the muscle in my back then when I would get the pain I'd have my wife or even one of the kids put their elbow on that muscle with pressure and it would release and then I'd be fine for a week or two until it came back.Just a thought I'm no doctor though get it figured out.
