Easiest and least costly would be a redrill of the flanges, but new axles would be the best way.
 
Don't even think about drilling the axles. Go buy the correct ones. Safety is #1
 
4 inch to 4.5 inch adapters ( ebay ) are the least expensive way IF... You have to be willing to run adapters given their bad history. They change wheel location about 1 inch minimum also which may be a factor for you.
Replace axles is the optimal/proper way but you probably know that... It isn't cheap.
 
300 new axles, 150 used axles.
200 for 10" or 11" brakes.
roughly, likely more.

Why are you going 5 on 4.5?

FWIW, My '69 axles were re-drilled back in the 70's. Screw in studs only on the stock flange. The 4' pattern was filled. For the 10 X 1-3/4 brakes the drums were re-drilled as well. Never had an issue with them.

Moser was selling stock length Abody replacement axles with the 4-1/2 circle or the 4" circle. A little more meat on the flange for press in studs.

Don't like the idea of re-drilling the axles or spacers.
Found the axles at Dr. Diffs web site and several other items of interest.
Looks like thats the way to go. I'll have to wait till the new year and order when I'm in Florida. Save some on shipping and duty.
Wish we had a Dr. Diff in the North.
