Youngblood, did you ever find a suitable choke thermostat for your application? I am in the exact same position, I have a manifold that looks exactly like your photo and haven't come up with a source either. If someone has a car with the same manifold, can you please post a picture so I at least know what I am looking for? Thanks!

Although there are several versions for the various applications, most can be interchanged. Most choke thermostats were electric assist, but a few applications were not and are listed in the foregoing list.
The following image shows a electric assist choke thermostat for a Thermo-Quad. The image is of the 1973-1976 version with the smaller red 3/16" connector. Later versions had a black 1/4" connector.
Non-electric assist chokes are similar without the heater coil and wire.
The choke electric control applies voltage to the choke thermostat electric assist based on ambient temperature and time. Four versions were produced.
- Chrysler 3656730, 3698357: 1973-1976 single stage.
- Chrysler 3755750, 3755770, Auto-Tune A32-901: 1974-1976 dual stage.
- Chrysler 4049799: 1977-1984 single stage.
- Chrysler 4049777, 4091034, Auto-Tune A32-903: 1977-1984 dual stage.
The following two images show the early and late style choke electric control. Both are dual stage. Single stage units are similar without the added resistor:
The 1971 - 1972 Thermo-Quad choke is the style that uses an enclosed well in the manifold, and is non-electric assist. Carter and Chrysler part numbers for this choke are as follows:
