thanks matt

Eye ball the alignment of the pulleys, close one eye and sight along the path of the belt - that sometimes can show you something odd if the pulleys are out of whack.

Was the old belt used for a long time ? maybe it snapped, or wasn't tight enough ?

I had the same issue and it was not related to pulley alignment. The taper on the v-belts available from many manufacturers today is two sharp. This allows the belt to lay over in the pulley. Once there is some wear the belt will twist and come off.

The belts from NAPA (I believe they are made by Gates) have the proper taper and actually sit slightly higher than the OD of the pulley. Since changing to the NAPA belts have not thrown a belt.
